To confirm dates we have discussed for your potty training appointment, please read and e-sign the Client Agreement and submit payment.
Please forward this page to any other legal guardians as well as anyone who will be present during the services (ie spouse, nanny, significant other, etc) to read & e-sign the DI Agreement before we begin any phone or in-home services.
​Payment can be made through Chase QuickPay or Zelle to info@nycpottytraining.com. If QuickPay or Zelle limits the daily amount you can send, partial payment may be sent to hold your date(s) until we receive the remaining amount the following day. A check for the remaining amount can also be mailed to the address at the top of the Client Agreement.
Please be sure to fill out the Intake Form as well so that we can develop your child's potty training plan and advise you on preparing for the potty training.
